Student Withdrawals- English form Spanish form
Please notify the campus registrar at least 24 hours prior to the student’s last day. The parent/guardian will need to show ID, complete and sign withdrawal papers and a leaver document. Textbooks, library books and any outstanding fees must be returned/cleared before the student is withdrawn.
MovingIf there is a change of address. Please provide a current utility (gas, light or water) dated within 30 days showing the service address, name of the bill holder, and a copy of the bill holder's driver’s license. If the bill is not in the parent/guardian's name, the Affidavit of Bona Fide Residency Forms are required along with a utility bill and driver’s license.Updating Contact InformationPlease submit these changes in writing or by emailing the campus registrar. Proof of Residence:
At the time of registration, parents will need to provide proof of residence (utility bill-gas, electricity, or water – not phone or cable bill) dated within the last 30 days with parent/guardian's name and street address. If a current bill is not yet available, a house or apartment lease agreement, or closing documents will be accepted. If a lease agreement or purchasing paperwork is provided, a utility bill must be provided within 30 days.
Please note: If the parent or guardian's name does not appear on the bill or lease agreement, then Bona Fide forms must be completed and notarized. Contact registrar for forms.
Immunizations:Proof of immunization must also be provided during registration. Student immunizations must be as up-to-date as medically feasible and must be certified by a doctor or public health official. A student will not be allowed to attend class if immunization records are not presented at enrollment. Katy ISD follows the Texas Department of State Health Services recommendation for immunization compliance in order for a student to attend school. The law requires that all students be up-to-date as medically feasible in order to attend classes. The immunization vaccine requirements for each grade level are listed on the Katy ISD website. Exemptions to the immunization requirements are allowable on an individual basis for medical contraindications, reasons of conscience-including religious belief, and active duty with the armed forces of the United States.
