ACCIDENTS – WORK RELATED

Injured Employees Must Notify Risk Management Prior to Receiving Treatment Unless the Employee is Unable or Incapacitated. If the Employee Cannot, the Campus Nurse or Supervisor Should Contact Risk Management. Example: Injured Employee is Transported Via Ambulance to an Emergency Room. Call: 281-396-2422 or 281-396-2212.

You Must Receive Treatment From an Alliance Network Medical Provider. If You Do Not, You May Be Responsible For All Medical Charges. Personal Medical Insurers May Deny Your Claim.


The following are procedures for a work related injury or accident. If an incident occurs while in the course and scope of your job duties, the following steps are required:

  1. Report the incident to your supervisor. Failure to report an accident or injury in a timely manner could result in a denial of your claim by the Texas Department of Insurance – Division of Workers’ Compensation.

  2. Report to the campus nurse if available, If not available call Lindy Shorthose at 281-396-2422 in the Risk Management Office, then Carol Moore at 281-396- 2785 in Health Services.

  3. Submit the online form “EMPLOYEE FIRST REPORT OF INJURY”. This form must be submitted to the Risk Management Department within 24 hours, (If you do not have computer access or are unable to do so, your supervisor is responsible for completing the process). **If a student(s) is involved in any way, DO NOT identify the student (s) on the form. It is illegal to include a students’ name on this form.

    Accessing and completing the online form from a Katy ISD work location:

    A. Go to katynet.katyisd.org– 1. Click Online Forms. 2. Type: Employee First Report of Injury. 3.
    Complete required fields. Print copy for yourself and one to take to the doctor the hit Submit.  ** PLEASE DO NOT SAVE PRIOR TO SUBMITTING OR EMAILING
    B. SUBMIT the form – The submit button is at the bottom of the form and will automatically send your completed form to the Risk Management Department. You will get a reply/confirmation once your report is received. **DO NOT SEND A HARD COPY TO RISK MANAGEMENT.
    C. Additional notes or information can be emailed to Lindy Shorthose.
    Please Note: If you need to send the form to anyone else, before hitting SUBMIT, go to File and then Send to Mail Recipient.

    If the injury is an emergency or life threatening, call 911 for transport to the nearest hospital emergency room. Then call Risk Management as soon as possible: 281-396-2422 or 281-396-2212.
